Wisconsin Lions  Youth Exchange Applications

We only accept the Electronic Format in Excel (.xls or .xlsx)

  Please remember to include:

•   Letter to Host Family;
•   Picture(s) of you and your family;
•   Copy of your passport;
•   Insurance information;
•   a completed and signed application form.

    PLEASE NOTE THAT MANY COUNTRIES HAVE A DEADLINE OF MARCH 15

    TO ACCEPT YOUTHS TO THEIR CAMP.

 The form on this page is used for applying to all Lions Youth Camp & Exchange camps around the world.

Please note that some camps will require their own specific form(s).  Please contact the Youth Exchange Chair in your area for assistance to insure the correct forms are processed. (see links below for assistance)
