•   Picture(s) of you and your family;
•   Copy of your passport;
•   Insurance information;
•   a completed and signed application form.

 The forms on this page are used for applying to all Lions Youth Camp  & Exchange camps around the world.  Please note that the electronic format is preferred but a paper form is also included.

Please note that some camps will require their own specific form(s).  Please contact the Youth Exchange Chair in your area for assistance to insure the correct forms are processed.
